August 23-24 and 30-31, 2008.

RE REGISTRATIONS ARE NOT COMPLETE UNTIL
YOU SIGN ALL NECESSARY FORMS AND PAY THE FEE
IN THE MAIN OFFICE.

Medical Release Form

We accept registrations for children of all ages for
Faith Formation on a year-round basis.
You may come by the church office to register and fill out
medical forms, or submit those forms online by clicking the links above.

Each grade level has it s own registration form.
Each child needs a separate medical release form for insurance purposes.

Parishioner Fees:
P3 to the 5th Grade $40.00
6th to 12th Grade $70.00
Scholarships are available.
